Long-term medical conditions

We have a recall system in place for all patients with one or more of the following chronic cardiovascular conditions which include patients with CHD (Chronic Heart Disease), HYPERTENSION (High Blood Pressure), DIABETES, CKD (Chronic Kidney Disease), HEART FAILURE, PAD (Peripheral Arterial Disease), TIA (Stroke) and also includes patients with RHEUMATOID ARTHRITIS.

In general, the majority of checks required for monitoring these conditions are the same so to try to make appointments simpler, we have grouped all these patients together under an umbrella title of “Long Term Medical Conditions.”

Many of these conditions require monitoring and the doctors have advised that an annual review is clinically sufficient and our aim is to get patients to the surgery for their “Long Term Medical Condition” review, along with any other review required for things such as ASTHMA, COPD, EPILEPSY or MENTAL HEALTH and even GP review for other medications at the same time.  We aim to carry out these reviews in the birth month for each patient to enable them to remember each year.

First Appointment (10 minutes) will be with the Health Care Assistant who will conduct blood tests, urinalysis and measure your height, weight and blood pressure.

Second Appointment approximately 1-2 weeks later (20-30 minutes) will be with the practice nurse who will discuss your results with you, conduct any further checks required for your specific condition and review and update your repeat medication.
